Parkwest Women’s Specialists accepts most major insurance and Medicare plans. Below is a PARTIAL LIST
ONLY of some of the major insurance plans accepted.
Please note that this list is subject to change and that even if your insurance carrier is not listed, your plan
may be accepted. Ultimately it is the patient’s responsibility to be familiar with their coverage and plan
requirements. Please contact Parkwest Women’s Specialists at (865) 690-7677 to confirm that we accept
your insurance plan or call the number on the back of your insurance card to ensure we are in network.
Currently, we are not participating in Medicaid plans such as TnCare or Amerigroup.

If your insurance requires a co-pay, we will request payment at the time of your visit. Be sure to contact our
office if there are changes to your insurance coverage.
We must emphasize that as a medical care provider, our relationship is with you, not your insurance
company. While the filing of insurance claims is a courtesy that we extend to our patients, all charges are
your responsibility from the date services are rendered. It is understood that temporary financial problems
arise; you are encouraged to contact us promptly for assistance in the management of your account. If you

Payment Options
We accept cash, Visa, Mastercard, Discover, American Express, and personal checks.
Card-on-File
We encourage patients to keep a credit card on file to make the checkout process easier, faster, and more efficient. You will no longer receive statements from us, but you will continue to receive your Explanation of Benefits (EOB) from your insurance carrier once your claim has been processed, detailing the charges and payments made on your behalf.
At check-in we will:
- scan the credit card of your choice, including your Flexible Spending Account (FSA) or Health Savings Account (HSA) card
After your insurance has paid their portion, we will:
- notify you via email of the balance owed
- charge the balance owed to your card on file
- email a receipt for the charge
Your credit card information will always be fully protected by our off-site, card-processing partner Elavon, and not on our computers, as required by industry standards (Payment Card Industry Data Security Standard – PCI-DSS).
Fees
Cancellation without 24-hour notice prior to appointment – $30.00
Missed Appointment – $30.00
Printed Medical Record – $15.00 (subject to change with number of pages)
FMLA / Short-Term Disability Paperwork – $15.00 per document
Self-Pay Please notify our billing specialists if you are a “self-pay” patient. A sliding fee will be put in place while you are registered as “self-pay” and payment options will be discussed and agreed upon between Parkwest Women’s Specialists and the patient or legal guardian.
